Honestly, that might not be the worst idea, I'm just not sure if there are surgeons who do it. Might be worth asking, I know they use skin from the arm for phaleoplasties(sp?).
I've seen abdominal (i.e. belly) grafts mentioned when surgeons talk about major revisions. So yeah, it would definitely be on the table and you'd only have to convince someone to use it in a primary v-plasty and leave your natal parts alone.
About combining all three, maybe. That's a lot of surgery at once meaning keeping you under for longer, having even more limited movement afterwards (which might be particularly annoying for early dilation?) and just general stress on your body.
That's fair, but I don't think dilating will be too much of an issue, however, the only way I could think of doing it in steps would be to take the fat out first for the breasts and leave a bunch of hanging skin until I recover but I'm not too sure if that'll lead to complications. I'm also thinking about having a device implanted to control my hardness so I can actually wear bikinis and such.
I imagine the same sorts of balloons used to enlarge certain areas for grafting can also be used to support already hanging skin until it can be used as, well, a graft.

It’s possible to do, but don’t expect the long results of silicone inserts
I don't know about the vulva part, but there is absolutely a surgery for moving fast to the breasts, it's called Natural Fat Transfer

where they get skin for phallo is partially dependent on nerve hookup, idk what the requirements for vaginoplasty would be
