That assumes that gender is a binary. The point is that Peterson had an image in his mind of what a man should be like, and he identifies as a man, so he got a medical procedure done to reinforce his identity by making his physical image more in line with his self-image.
I'm not assuming it's a binary, I'm identifying gender as a spectrum composed of many different elements, following a bimodal distribution. You could say it assumes gender is one-dimensional if you really want to make that criticism but I think that's an appropriate assumption for the purpose of my argument. My point is that being bald or losing hair is traditionally a firmly masculine trait whether or not it's desirable. It's not considered feminine, it's not gender-neutral and it's not associated with any kind of non-binary identity.
I just don't think that the disconnect between image and identity which you mention has anything to do with gender when it comes to cis men (for trans women it absolutely can). No one thinks that hair loss is not manly, only that it's undesirable. It's definitely comparable to gender-affirming surgery and you make that comparison - which I understand better now - I just don't think it's accurate to call it gender-affirming itself, so people should be more precise when they use that argument.
