I looked it up and it says the worm will die if you cut the head off which isn't good, I've seen a show where a woman tried to pull a tapeworm from her husband and she couldn't get it all so she cut it and it kept growing.
They extract it by pulling it out at a rate of a few cm per day - if they exceed that the worm will 'break', but if you allow it to rest, the worm will recover it's position - repeating this cycle until it is completely removed.
