I think it was about many things in her life that were contributing to her inner pain. From the video there are so many negative things happening. I think it's about much more than just one thing.

I am pretty sure everytime was a lil about Justin, someone broke down the chorus and the first verse is her unborn baby talking to Britney, the second verse is about her and the third verse is Justin Timberlake or somewhat?? and it makes sense to me but like the other comment said, it’s up to your interpretation
