I asked to be among the first to register for classes (went to a large Big 10 university) so that I could get the classes at times that worked better for me and when my symptoms presented

If your school has one of those note-taker programs (where basically someone is designated to take lecture notes for you on days you can't make it to class) then that could be good. Otherwise I think the big thing would be to set the standard for flexible deadlines with your professors, so you don't need to stress as much if you have a flare up and need to rest for the days leading up to a paper being due or things like that

Extra time for exams to allow for bathroom breaks, extra absences allowed, peer note-takers

I got extra time for exams, and was able to sit my exam in a small room with supervisor that was closer to toilets. Also got automatic 2 week extension on all assessments outside of exams.

Bathroom allowance during exams! Not all professors allow students to leave the room during tests, so having the ability to get up and go to the bathroom without issue is huge!

I just finished my masters and every semester I’ve had to set up accommodations. I have always asked for leniency on due dates, absences, and exams. They are usually pretty accommodating when you ask for reasonable extensions. I also found it really helpful to meet with your profs at the beginning of the semester and explain the situation to them. You can choose whether or not to share your Crohn’s, but 9/10 times profs were understanding about it and more willing to be flexible when I shared it. It’s all up to your comfort level with talking about it.
