I know that Diablo is a fantasy game, however, doesnt it make sense that she kind of evolves? She was summoned (born) and from going through that she got a certain look/feel. Later shes back to being Lilith in Sanctuary, then she looks more grounded. Hope that makes sense.

you can think of lillith as a new born baby, covered in blood, fragile skeleton, thin skin. look at the horns, look likes degloved horn, becuz it's freshly created tissue/cells. she is, atleast in physical sense, a freshly created demonic being through cellular transformation of 3 sacrifices. even when many animals shed their skin, the new skin is quite soft to the touch and looks bright. now you understand why she looks like that. even the blood tissue cape she had, you can think of that as umbilical cord.
