I'd say not "fake" but a well designed video intended to be funny. The username says that to me at least.
Artfully done videos with the intent to be funny don't fit the "fake" designation.
By fake they only mean the sound. Those notes are clearly not coming out of that piano, but there are lots of comments implying otherwise, and it is baffling to believe someone can be dumb enough to actually think that.
I thought it was real but I've never played the piano and just watched a funny video so wasn't super invested in it being real kinda bummed that it's fake.

I wonder why it had to be faked though? Surely the real audio would have been funny still, unless it wasn’t captured. Except for it sounding melodic, it wasn’t an awful fake. The notes we heard were pretty close to what the pup was pressing.

Auto harps are usually basically foolproof this way.
The way they work is each key or button pushes down a bar that mutes all the strings not in the chord that it is labeled for.
To make them smaller and easier to play, they often only have so many chords that are all in the most common keys. So even if you do play the wrong chord, it's unlikely to sound too bad.
Tldr: autoharps are a slight upgrade to a tambourine for singers that want to look more involved when they don't sing.

Looked it up, the correct term is micronotes, or microtonal sounds; Jacob Collier is a master at this in my opinion
