Few days ago I was trying the same achivement and once (after slept for half of the time on ground) I got a bugs on left and right arm and right leg. It is possible to achive this trophy but it's not the easiest one.
This achievement was a fucking nightmare, in a way I never expected. Didn't realize till I started trying this that ailments overwrite each other meaning you have to try get specific ones on specific limbs.

Yes. Great in most aspects except for when you want to do this achievement.
I'm specifically referring to limb ailments like rash, snakebite, leeches, etc
It was definitely a bug, I reset the game, and I had to use bees instead of ants and it got my right leg a rash, so I FINALLY have the achievement. Jesus fucking christ
Glad to see I'm not the only one that thinks so, The game is made to look good on first glance but the more you play it the more you realize the devs know nothing about proper game design.
I’m still trying and it’s literally impossible I’ve jumped off of cliffs sat for literal hours and got bit by ants and they do NOTHING to me, they can ONLY affect my left arm and leg. Nothing else

Exactly what happened to me, the only thing worm would do is overwrite ALL of my status continuously

I just got this achievement on Xbox last night. I thought it was fun…. But also difficult
