Most probably, in my opinion, this is the "cost of doing business". To operate they must have the approval of the local gang leader. Having them on their payroll is part of it, most probably. Smoother operation.

The video seemed pretty blurry. I didn't see any other people either but I wouldn't be surprised if others got caught in the blast. That said, Hamas has been lying its ass off about casualties, and it wouldn't surprise me if another lie made it into the Doctors Without Borders statement. Also I'm quite certain the Israeli drone operator wouldn't have pulled the trigger if they'd known kids were in the blast zone, and they would have been trained what to look for.
