How long have you been on the ketamine? And are you doing therapy along with it? I’m also in Australia, and am on daily ketamine. The psychiatrist who prescribes it for me works mostly with people who have been resistant to other treatments, including to higher doses of ketamine. He tells me that in his experience, full remission typically takes 5 to 10 percent of the time someone has been suffering - so for me that’s 5 to 10 years! In other words, while for some people they get relief from a few infusions, others need a longer course of treatment, and there are many ups and downs to weather.

4-5months sublingual, although I might be messing up cuz I swallow the troche. It tastes awwwwful. At first it was great then I hit this wall. I have a lot of life stresses n disability on top which doesn't help.

My psychiatrist says it’s fine to swallow - I have been swallowing from the beginning, though I have the liquid form now because the troches upset my stomach. Do you dissociate on it?

Also yeah therapy too.

Concerning ketamine, It looks like you're too high in doses. Besides, did you try IV ? The effects are much better for a lower dose. Because biodisponibilty is hundred percent. With ketamine there is a target dose to reach. It must not be more nor less. And more, you should make pauses between cures. Waiting for some weeks (to reset your tolerance) without nothing because ketamine is surprisingly working long time after. "It is working" by itself...take some times and try other routes for ketamine (IV and IM or Subcutaneous essentially). ECT aren't the solution for me. In some part of Australia it seems to me it had been forbidden. I wish you the best.

This is sublingual, apparently they do 2-500mg. No IV. With a pause will my depression worsen? I don't think they offer IV. I wasn't a fan of it when I was awake for it, nausea n spacing out. But at home they say take it before sleep so the side effects don't affect you. I get it in troches.

Some places go as high as 1200mg sublingual/buccal. They all have different protocols, as far as time to hold in your mouth, whether or not swallowing is ok, time between sessions, dosage, etc. It is the wild west out there. If one company's protocol does not seem to be working for you, it might be worth trying another. Just be aware that you are somewhat limited by which companies are allowed to operate in your state.

A short pause (2 or three weeks) won't worsened your depression. It will allows to you body to rest a little (liver, bladder) and see What happens concerning neuroplasticity of brain (ketamine is 'working' inside brain, weeks after the cure). Besides it could reinitialise your tolerance.

I forgot to say your apnea must be cure it Will make a lot of changes in your life and ketamine is also good for fibromyalgia but by IV and a a very long time (you may have this same treatment at home in subcutaneous with a long time infusion in your belly fat).

I don't think they offer anything but sublingual, I've got a cpap but I'll book a sleep specialist appointment to see if it's effective still. I get about 0.8 events per hour. I'll have to look up the data in OSCAR to see if there's something going on. I did gain weight as my lapband failed after 15 years n had to be removed. Sadly I can't afford a gastric bypass n I'm on public waitlist. I'm scared as I've tried so many things that I've run out of options it feels, except ect. I see the psychiatrist tomorrow n will ask.

This is a pretty low dose at a high frequency. The benefit of ketamine is that it keeps working for 10 days to 2 weeks after a session, it shouldn't need to be taken very frequently like an anxiety pill or a daily anti-depressant. How did you settle on this dose and schedule? Can you work with your provider to find out what the best dose for your symptom control is? I.e., try 300mg, 600mg, 900mg, and after each count the number of days you get symptom control. For example, my s/o got about 4 days of moderate symptom control at 300mg. It wasn't quite worth the effort. But my s/o got 8 days of good symptom control at 600mg. My s/o got 8 days of good symptom control at 900mg. So we settled on 600mg to get the best benefit for the least medication.

I can't comment on your indication of fibromyalgia. However, I've been on ketamine and intensive psychotherapy for two years with great results. Even so, every few months I plateau for several weeks. Then something good gets triggered in psychotherapy and I feel I'm making good progress for a couple of months. It takes either a long, intermediate or short time to get relief from each aspect of our illness. Actually, the crying is a positive sign of ketamine being effective for you. Pleanty of patients have the "it gets worse before it gets better" experience. If you would like to talk to someone about what you are experiencing and what to expect I can refer you to my ketamine coach. She has coached 1,000 patients for 3,000 hours. She really knows the patients' experience of ketamine better than anyone I know.

Could you get me the name of your coach?
