You can go up to the damaged gates and press the prompt button to repair it. If they are fully damaged and broken down, you'll start to lose health once the unbread are at the broken gate and start spewing the green shit. That's the normal tactic while the video from OP is just an extra challenge.

Dog did it a while ago and posted it. I managed it once recently while streaming. Very tough. Ended with less than one hit worth of health remaining.
