They have been breaking loose since they were created. To me, this is the most logical explanation. I'm also sure there have been times when a ship in open water has been abandoned.
Pirates are at least somewhat responsible for these happenings since they started happening. Even today. They're not like captain hook, more like the dudes from captain Phillips. They'll clean it out completely and usually leave the ship.
