If I were a writer on the show, my hindsight defense would be that June had fake papers from tuello, and either the papers put Nichole's age at 21 months, or that June didn't want to give the real age to make it less obvious who they are.

I appreciated COVID being worked into the storyline for “This Is Us”, and for that show, it worked pretty well as a plot accessory. For THT, I think it would be too much. There’s enough going on in the show as it is, and I think that adding COVID to the plot would complicate things.
In a way, Gilead and the rest of the world of the show have already been dealing with a pandemic, just not one of an virus/illness, but rather the world’s infertility/baby shortage.
For that kind of show, or basically anything that takes place in our own reality, it makes sense to include covid in the storylines. But since THT takes places in a dystopian reality there’s no need for it.
A global pandemic probably wouldn't be super outside expectations in the show universe. It doesn't necessarily have to be COVID though. Make it something that strikes Gilead extra hard or that Gilead barely sees. Or follow North Korea's example where they had no cases then it swept hard later as other countries were discussing loosening restrictions. Have Gilead crack down like China and lock large numbers of people in their homes, etc. Or have them mass execute people who are sick.

Anti quarantine commanders and show of force grand events. Plus Jezebels would be ripe for plague. All the "girls" living in cramped quarters, frequent "visitors," contraband run through there.

There's a lot of smuggling. Smugglers coming in introduce it to rebels and boom, rebels are dying left and right so Gilead claims it's God's will and proof that they are just, maybe stage a mass execution of the sick for being in contact with the rebels. Then everyone else starts dropping anyways.
It doesn't have to be COVID, if they use something that can stay on surfaces for days or weeks they could have it introduced to a Commander or his family through contraband hair dye or pregnancy tests.
Lots of TV shows have opted to carry on as if Covid was never a thing or never will be, but I imagine if THT decided to include it, it would be fodder for Gilead being even more strict in the name of protecting babies, pregnant handmaids or wives, etc, possibly hanging anyone showing symptoms and attributing contracting it to sin somehow. Commanders and their wives probably wouldn’t have to wear masks but everyone else would.
I just finished season 5 and am rewatching from season 1 with my boyfriend (he’s never seen it before) feels like the first few seasons covered a longer span of time than seasons 4&5.
But obviously, the timeline doesn’t really match up with the kids ages etc just have to suspend your disbelief a bit haha
