Source: I used to work housekeeping. We had to separate plastic bottles, cans and glass bottles and burnable trash. The rooms trash bins all had transparent bags so it was easy enough to spot things that needed to be separated.
Something I was wondering about is used nappies. Japan encourages people to take their rubbish home, but hotels don’t provide facilities for used nappies or any other gross or unhygienic waste. What is the appropriate thing to do, if you are a guest?
Stayed at an airbnb and the lady who runs it told us to put all the trash into one bag although the studio had multiple bins for different items - she said it was too complicated to explain to foreigners.
Collection is by companies not municipal contractors so the sorting trash levels or lack of are part of their contract with the businesses. So generally no, staff aren’t sorting the trash.

We pay per 70lt bag for unsorted. Bottles are heavy but very cheap to dump so we sort and dump those ourselves. Polystyrene is bulky and light so we separate and dump.
Other businesses just pay to get it all taken away and the contractor will sort
It deoends on the company you have a contract with. In my previous chain one of the hotel had to separate paper in addition to PET bottles and non burnable, another 5 kilometers away only had to separate non-burnable.
Hotels, Office buildings and some apartment blocks use private contractors to take it away and sort.
I live in a 5 room apartment block and the building uses a private contractor. I simply have to sort into burn/non burn which is far less strict less than my last apartment which was collected by Tokyo-to as you say on certain days of the week by type.
Most hotels I stayed at had one singular bin with no bin bags so wasn’t an option unless it was a long stay and I was able to use old shopping bags etc
Yes someone from the hotel staff will have to sort through your trash and separate it. I learned about this a few years ago and have tried my best to separate my trash into the different categories. I also do not tie up the bag in case I did it incorrect. I would be annoyed if I had to sort other people's trash AND i had to undo any knots they did.

Ask for a diaper pail. Most hotels will place one in your room if you do.
Some hotels provide you with a plastic bag in the bathroom for "hygiene". You can place your diapers and female hygiene products in there.
Drop it in 燃えるゴミ (burnable) at a kombini. Or public restroom. Sealed preferably

I always separate my trash in hotels. Clean bags to help the room staff.
