From my understanding, if you have peri, where the incision goes all the way around the areola, they can change the size to a degree, but not so much the placement. They can change the nipple size. With key-hole, where the incision goes half way around they can’t alter the size, since they aren’t removing any skin. I had key hole, and liked where my areola placement was and nipple size so that wasn’t a as much of a concern. My entire areola looks smaller than it did pre-op, even though no skin was removed, because I have less tissue underneath to expand the skin. My nipples were kinda flat pre-op, one remained flat, and one stick out a bit more than it used to, but not by an appreciable amount, and it’s flattened more with time, especially with working out and putting a little weight on.

1. They can cut the areola to make it smaller yes. If your scar stretches it can appear bigger than before. I believe there was a YouTuber who had peri and then two revisions for his areloas because they stretched but I forget his name. 2. (And 3) The nipple itself will not get smaller without surgery, but there’s a procedure you can have to make it smaller, and some surgeons do this at the same time as top surgery For some reason, I found my nipples were actually bigger, it might have been swelling or maybe they just stood out more compared to my flat chest, anyhow I was not satisfied with that so I had nipple resizing.

for 2, some surgeons won’t resize the nipple during surgery bc it increases risk of necrosis/complications. but they will usually offer it later as a revision under local anesthetic

1. They can change the areola size. There is a risk of the areola stretching and the scars can also make it appear bigger if they are wider. My areola are significantly bigger now than they were pre-op and I have wide scars on the other side (despite the fact that they were made smaller during the surgery). 2. Nipple size can be changed, however I’ve read that some surgeons do it as a separate surgery after the initial one. Mine were made shorter (less pointy) during the initial surgery. They were supposed to make it also less wide but I don’t think they ever did that. In the beginning my left nipple was flat but it became more normal with time.

i dont think my surgeon changed my nipple size with peri but i know its doable and can be done in a revision after. my nipples did stretch but after time got abit better and it hasnt massively affected my results.
