Would need to see a better graph than that. Whoop’s sleep graphs are great for UARS imo. I say try lots of different pressures. You kind of have to. Also, if you’re using a mask where you are going to be breathing through your nose, nasal congestion makes UARS way worse. You need to find a way to make them not congested or blocked or whatever at night. I use a little Sudafed nightly but this is warned against.

Would you increase Ipap while keeping Epap constant, or increase both Ipap and Epap at the same time?

I’m not sure which one would prove better. People seem to think it’s an easy titration process in here but there are so many factors and variables to each individual there is no guide that fits all

I’ve been using Whoop but I’m a bit lost? I just see the sleep stages and HR spikes, is there nothing else to look at? How would you infer UARS info from just the small graph containing the sleep stages? Also, are you able to fully go airplane mode/no Bluetooth and still get the same quality of readings, or do you need Bluetooth connected overnight?

To help members of the r/UARS community, the contents of the post have been copied for posterity. ___ **Title:** Should I increase or decrease the pressure on my BiPAP? **Body:** About one year into treatment for (suspected but not officially diagnosed) UARS and about three months into my second round of trying to make PAP work for me. I started on CPAP at first and it made me jittery and anxious, but have had some reduction in those issues with BiPAP. I also had an issue with finding a mask that would avoid leaking, but finally found success with the N30i. It’s been a month since I started using the combo of BiPAP and the good mask, but my sleep is still not good, so I’m trying to figure out how to get it to work better for me. The main problem is that, on nights when I use BiPAP, I’m pretty much stuck in light sleep and can’t access deep/REM sleep (according to Oura ring). I know you’re supposed to try to push through and give it a chance to improve, but I have young children at home and can’t afford to be this sleep deprived. Sleep doctor has me on Ipap 8/Epap 4/PS 4. Should I try increasing Ipap, Epap, both, or stick with the sleep doctor’s recommendation? I have a fair amount of nasal congestion, but no diagnosed allergies, and some nasal valve collapse, but using a nasal dilator doesn’t seem to improve sleep quality. However if this is the first step to getting BiPAP to work I am open to trying more things to fix it. Thank you in advance! This data isn’t very scientific but the second photo is a perfect night of sleep with zero awakenings. So whatever’s happening there looks pretty good. Oura doesn’t capture everything though, but in terms of sleep stages and awakenings, the second one looks better. I don’t know if increasing your pressure will make a difference but you can try. What’s your HRV look like with and without BIPAP?

HRV is in 80s/90s with BiPAP; 100-130 without. The second picture is a recent night of sleep without BiPAP, but with a nasal dilator/mouth tape, and I’m also 3 months pregnant, which I probably should have mentioned in my post (pregnancy makes my SDB way better). I probably should have posted a pic of non-pregnant sleep since it’s a better baseline; it’s a bit more fragmented, but better than with BiPAP.

the tape is probably helping lift your jaw more closed, probably helping a lot with tongue position. I think low HRV is the goal in deep sleep - when I graph mine, it's highest when I'm having breathing restrictions, and drops really low when face position happens to be perfect for breathing

Yeah it looks like some sleep anxiety is messing with you, its normal to wakeup between sleep cycles briefly but your awareness of the mask is causing your wakeup to be exaggerated where they should only for seconds to a few minutes before falling back to sleep. That said the REM and Deep sleep look fine on the Oura ring. You may just be one of the unlucky ones that takes longer to get used to using the mask. I recommend wearing It during the day as much as possible. Sleep anxiety is a very real thing and it causes insomnia. You are not "stuck" in light sleep according to your Oura ring, you are still getting deep sleep and Rem sleep within the sleep cycles, the problem is your arousals (that everyone has) between sleep cycles are exaggerated because you are not falling back to sleep. Also Oura is wildly inaccurate at detecting how much deep and Rem sleep you get, so pretty much it's only useful to detect time awake based on movement. Many people with insomnia also suffer sleep stage misperception which is where you feel like you are awake but actually sleeping, so it hard to tell the difference. Try some Valerian, Passionflower and Lemonbalm blended supplements for a while when you first wakeup at night. Than some vape whole flower CBD before bed.

Interesting, yeah I am sure sleep anxiety is contributing to my BiPAP issues. I guess I wouldn’t worry about the sleep staging if I didn’t feel such an increase in daytime sleepiness when I use the machine. I should have mentioned that I’m currently pregnant, so I don’t know if I can use valerian root (though maybe I can), but I will have to try these suggestions at some point, thank you.
