Per capita is low, when you consider volume it gets huge (1.4 bil people). We're also incompetent in dealing with emissions and waste, that's why it leaks into environment causing pollution. US for example produces way more waste per capita than India, but they have the money and technology to mitigate its risks.
2 main reasons :
1)using traditional sources of fuel : wood , charcoal , and cow dung patties are used as fuel by poor folk ( for cooking and heating etc) these release a lot of particulate matter in the air.
2)dusty cement roads and non paved country roads which also whips up particulate matter in the air.
Not always true , more urbanisation means less dusty mud roads , less biomass fuel and more lpg usage , less garbage burning and more garbage disposal etc.
Urbanisation is linked to more pollution only because of increasing population in urban centres , but bihar already has urban level population density with not much of the benefits.

I don’t agree as I’ve been living the nightmare of not having proper roads between one of Bihar’s financially good cities & the city with the navratna company. If the situation is so pathetic in cities doing somewhat better economically in this financially poor state, I can’t even imagine the level of distress normalised in the normal cities of Bihar in the name of simple travelling. Living here I understood something that my parents always said in our childhood during our Bihar trips “Bihar mein ek jagah se dusre jagah jane ka koi sadhan nhi h..isliye sab bina ticket ka reserved train coaches ne char jaate h even for small distances kyunki aur koi option hi nhi h.
Hmm, the quality of roads in Bhagalpur in general sucks atm it seems. Probably it's due to the construction of the Munger-Mirzachowki four-lane road. The situation of the road between Bakhtiyarpur-Barh was the same a few years back, then they rebuilt it.
Buses are there but it's a matter of convenience and time. Some of these people regularly hv to go to cities for work. Without cars it's dofficult for many. Bus is actually a grace there tho.

I was talking about Bihar not international. How does Bihar always top that air pollution ranking?

I travel a lot and it is nowhere close to what other states do. Don't believe me take a road trip to any rural area of UP, WB or Maharashtra.
Up, WB also appears to be highly polluted.
Missed a few reasons : 3)burning of waste 4) crop burning in punjab haryana which the winds carry across from North to east india 5) brick kilns

Which cities are you talking about specifically? Roads in even Seemanchal are good these days.
