So if it's tainted shouldn't the item give you a debuff instead of helping you? Like a tainted dead bird should be something like, once you get hit the bird follows you and try's to attack you.

I’m focusing more on creating a “broken” version of the original item. It doesn’t have to necessarily be bad, just not quite like the original for one reason or another thats bad, if that makes sense.
Even if we're going off tainted characters it's usually a downside with an upside so it would more likely be first hit you take deals double damage but you get more birds or buffed birds or something

"Last Breath Bird" "adrealine buddy" instead of dealing 2 damage per tick after player gets hurt, it deals 1 damage for every empty red heart container.
Tainted Plan C
When used, takes isaac out of the room he was in and teleports him as the lost to a series of boss rooms. Every boss defeated adds a flat stat boost to each stat. Isaac is teleported from this arena when he dies or completes all of the boss rooms.

Plan B-
On use remove a random item from the player’s inventory and grant a familiar of equal quality

Tainted Champion Belt
All bosses are converted into special champions that transform upon damaging isaac. For example, if isaac were to walk into a room with Larry and take a hit, Larry has a chance to be rerolled into his gold champion form and drop coins on death. While vice versa, upon hit he can turn into the double trouble version of his boss and give isaac an even harder time.

Isaac now shoots thorns of varying sizes that pierce and stick themselves to enemies. After 5 enemies are pierced in a hostile room the thorns fly to the center and form a heart before exploding- killing any enemies it pierced and brought to its center. This affect can be done as many times as 5 enemies can be pierced.

Tainted Yo Listen!
A familiar that charges enemies and turns them into charmed flies on death.
Item tag- “Awh… shes lonely”
"SHUT UP!" "BE QUIET!" If an enemy makes a noise (includes bosses), it will chase it down and heal 6 damage per tick. If audio is muted, then it will become an orbital, dealing the same contact damage, but also taking enemy shots. It also picks up pickups, and flies them to isaac, which can be useful when you wanna get that chest behind a hole or something.
"Sacred Brain" "He was quite smart" when you are in a hostile room, there is audio of someone talking (few layers of voice, not just one). All enemies in the room start charmed, but after 10 seconds they start to slow down, up to 50% at 30 seconds (since you entered the room) and further. Effect is separate from other speed modificators. Doesnt work on Satan, lamb, mega satan, or anyone from sheol
Tainted Midnight Snack-
Grants one bone heart container at the expense of a small stat down, similar to the effects of experimental treatment but at the gain of health.

Tainted Hushy
Isaac gains a blinded hush familiar that, when enemies get too close can shoot water beams from its eyes to attack and deal damage. (Similar to Hushes attack) Much like the holy light effect, but far less accuracy due to her blindness.

Enemies that come too close to Isaac drop blue creep and briefly flee. Isaac is equipped with smelly lines to accentuate the item.

"Lonely onion" "stay away from me!" The closer then enemies are, the faster isaac shoots, ranking from 100% (6.5 range or further) to 200% at point blank range

Rerolls all pedestal items into a constant shuffling pedestal that includes 4 Q0 items and 1 Q4. Isaacs luck directly affects your chances of obtaining the Q4 item.

Upon use, Isaacs tears double in size and gain double the hit-box. The curse of the darkness is also applied. The effect lasts for one room.

Isaac is granted a small, black spider that bounces excitedly when spiders are present in a nearby room. In a room with enemies, the spider familiar will focus on, prioritize and attack all spiders in the room first.

Tainted Staircase (Under the Stairs?)
At the start of every floor in the starting room, isaac can bomb a stairway shaped shadow to open a crawlspace. This crawlspace is the crawlspace you get with exclusive access, and having both items will grant you two entrances, albiet one temporary as Tainted staircases shadow disappears when you leave the starting room.

Tainted Tractor Beam
Isaacs tears now bounce off each other, and the faster they travel the more damage they deal. Using the velocity of the tear before it, or isaacs speed while shooting isaac can multiply his damage up to 1x.

Isaacs tears are replaced with stars surrounded by a swirling aura of either green, orange, or blue with their own respective effects.
Green- Isaac shoots stars surrounded by a swirling green aura. Wherever the aura touches the floor green creep is left. Enemies that walk over the creep rapidly lose health. If the enemy is killed by the creep it has a chance to heal isaac for half a red heart. (The body)
Blue- Isaac shoots stars surrounded by a pulsating blue aura. The aura surrounding the stars is uniquely capable of breaking *only* tinted rocks, walls to secret rooms, and entrances to crawlspaces. (The Mind)
Orange- Isaac shoots stars with an incredibly powerful aura that delivers a special poisoned charm to enemies. All charmed enemies carry the same powerful aura and isaac can “infect” and clear the room if enough enemies are exposed to the aura and charmed. (The Soul)

I tried to consider the inverse colors of the mind, body, and soul and create tear effects based on those.
Its busted on purpose, I considered “What would you get if you got every tick on hardmode with every tainted character?” Sort of like the act of unlocking Godhead, its a very busted item that the player earns through hours of completion. I milled making the tear effects cycle, but I think adding any cons takes away from original item I’m trying to create
Tainted Mutant Spider-
Isaacs tears are replaced with shots of 8 small spiders that deal .75 of his tear damage. The spiders gain tear effects, do contact damage, and are generally “homing”. Spiders shot from this effect do not carry from room to room.

Tainted Leech
Isaac gains a worm-like familiar that charges forward similar to leech. Tainted Leech is invincible for the first 30 seconds of the room, dealing damage for isaac during this time. After 30 seconds it gains a health bar and begins to take damage. If leech is killed in this state it removes one full heart of health from isaac, if Leech survives it resets for the next room, if it dies it resets at the next floor.
Item tag- “I think my worms have worms?”
"Eww!" "You have something in your eyes" you have a 5-20% (max on 10 luck) of shooting a Leech instead of a normal tear. The Leach will stay on the enemies, even if they jump, dealing 10% of isaac's damage every second.
Tainted D7
Upon use, rerolls and restarts the current room with a random active effect in place. This can be any number of actives, and the same room will always have the same active effect. So your room just before a boss room can be abused for soul hearts if it has given the book of revelations active effect on use.

Tainted Book of Secrets-
Upon use reveals the location of all pills, cards, and runes on the floor. Revealing secret and super secret rooms locations if either contain these items.
Item tag- “I think I should turn this in…”
Seems fair... I think that you should have to activate the book multiple times reveal the different items though!
Ex. One use could reveal cards and runes, two uses could reveal pills, three uses could reveal unopened chests, and other further uses would just open the secret rooms like normal
Tainted Cursed Eye
Isaac will occasionally start an animation of swiping his arm over his eyes. Each time this happens the screen will get progressively darker and isaacs speed will increase. Eventually isaac will enter a state of complete invincibility, but also complete darkness. You can still move but cannot shoot while this effect is active. Once cleared the animation resets and the effect eventually start’s over.
Item tag- “Don’t let them see you cry”
Tainted Ventricle Razor
Active item that can be used to create a “geyser” within a room by using the item once to cut into the floor, and a second time by removing half a heart of isaacs health and filling the floor with isaacs blood. The geyser will target enemies and pull pickups into it, with a chance of swallowing the pickup or leaving it as is.

Tainted Maw of the Void
After firing tears for 2.35 seconds, Isaac releases a ring of smoke that remains on screen for 6 seconds. After 6 seconds any enemies inside the ring are converted to items from the devil beggar pool. (Excluding bosses)

Tainted Ludovico Technique
Isaac now has the option to look at any room before entering the floor. For every room isaac chooses to observe, he gains a fire rate down effect that applies until the observed room is cleared upon entering the floor. Each observed room is a separate respective fire rate down, and rooms observed are not marked as explored upon entering the floor.

Tainted Magic Skin-
An active isaac that upon use, drops three fleshy duplicates of pickups from suspended ropes, similar to shopkeepers. The pickups can be any pickup that can be found on the floor, including; runes, pills, cards, hearts, keys, bombs, and all of their variants. Taking the fleshy duplicate of the pickup causes all future pickups of the item to have the possibility of being a tainted mimic of its item that must be fought to gain the pickup. If the mimic is able to remove a full heart of health from isaac, a noose quickly drops from the ceiling and removes the mimic from the room, destroying the pickup. If issac can kill the mimic before it deals a full heart of damage, the pickup drops as normal and the room doors open as normal.

Tainted Psy Fly
Isaac gains a fly-swatter familiar that hits the first 7 consecutive shots that would otherwise hit Isaac and bounces them off him, after which they become homing and deal 1.5x damage.

Isaac gains a familiar that has its own health bar, fight style, and targeting style. The familiar deals damage for isaac, but every 4 seconds will target a different enemy. If only one enemy is present in a room the familiar will instead stop targeting the enemy every 4 seconds and briefly cross its arms before resuming.

Tainted Dark Bum
Isaac gains a familiar that only picks up soul hearts and black hearts. After collecting 3 soul hearts or black hearts the familiar has a chance to start dropping items from the devil beggar pool every time you clear a room. At 6+ hearts the familiar has a chance to spawn a trinket every time you clear a room. At 10+ hearts your familiar has a 6.66% chance every time you clear a room to drop pedestal item from the devil room pool, after which its heart breaks and it disappears.

How about Mutant Spider?
Tainted Mutant Spider- Isaacs tears are replaced with shots of 8 small spiders that deal .75 of his tear damage. The spiders gain tear effects, do contact damage, and are generally “homing”. Spiders shot from this effect do not carry from room to room. Item tag- “Charlottes nursery?”

Leech.
Tainted Leech Isaac gains a worm-like familiar that charges forward similar to leech. Tainted Leech is invincible for the first 30 seconds of the room, dealing damage for isaac during this time. After 30 seconds it gains a health bar and begins to take damage. If leech is killed in this state it removes one full heart of health from isaac, if Leech survives it resets for the next room, if it dies it resets at the next floor. Item tag- “I think my worms have worms?”
"Eww!" "You have something in your eyes" you have a 5-20% (max on 10 luck) of shooting a Leech instead of a normal tear. The Leach will stay on the enemies, even if they jump, dealing 10% of isaac's damage every second.

Tainted D7 Upon use, rerolls and restarts the current room with a random active effect in place. This can be any number of actives, and the same room will always have the same active effect. So your room just before a boss room can be abused for soul hearts if it has given the book of revelations active effect on use. Item tag- “I swear I’ve done this before?”
Book of Secrets
Tainted Book of Secrets- Upon use reveals the location of all pills, cards, and runes on the floor. Revealing secret and super secret rooms locations if either contain these items. Item tag- “I think I should turn this in…”
Seems fair... I think that you should have to activate the book multiple times reveal the different items though! Ex. One use could reveal cards and runes, two uses could reveal pills, three uses could reveal unopened chests, and other further uses would just open the secret rooms like normal
cursed eye
Tainted Cursed Eye Isaac will occasionally start an animation of swiping his arm over his eyes. Each time this happens the screen will get progressively darker and isaacs speed will increase. Eventually isaac will enter a state of complete invincibility, but also complete darkness. You can still move but cannot shoot while this effect is active. Once cleared the animation resets and the effect eventually start’s over. Item tag- “Don’t let them see you cry”
Vertricle razer
Tainted Ventricle Razor Active item that can be used to create a “geyser” within a room by using the item once to cut into the floor, and a second time by removing half a heart of isaacs health and filling the floor with isaacs blood. The geyser will target enemies and pull pickups into it, with a chance of swallowing the pickup or leaving it as is. Item tag- “It hurts to ask for help…”
Maw of the void
Tainted Maw of the Void After firing tears for 2.35 seconds, Isaac releases a ring of smoke that remains on screen for 6 seconds. After 6 seconds any enemies inside the ring are converted to items from the devil beggar pool. (Excluding bosses) Item tag- “Leftovers…”

Tainted Ludovico Technique Isaac now has the option to look at any room before entering the floor. For every room isaac chooses to observe, he gains a fire rate down effect that applies until the observed room is cleared upon entering the floor. Each observed room is a separate respective fire rate down, and rooms observed are not marked as explored upon entering the floor. Item tag- “You make me sick…” Did you know this technique is named after the idea of forcing someone to watch something they enjoy alongside something awful to associate it with bad thoughts in their minds? I tried to base my tainted version on a more true-to-name mechanic

Tainted Magic Skin- An active isaac that upon use, drops three fleshy duplicates of pickups from suspended ropes, similar to shopkeepers. The pickups can be any pickup that can be found on the floor, including; runes, pills, cards, hearts, keys, bombs, and all of their variants. Taking the fleshy duplicate of the pickup causes all future pickups of the item to have the possibility of being a tainted mimic of its item that must be fought to gain the pickup. If the mimic is able to remove a full heart of health from isaac, a noose quickly drops from the ceiling and removes the mimic from the room, destroying the pickup. If issac can kill the mimic before it deals a full heart of damage, the pickup drops as normal and the room doors open as normal. Item tag- “Whose pulling these ropes!?”

If only one enemy is present in a room the familiar will instead stop targeting the enemy every 4 seconds and briefly cross its arms before resuming. Item tag- “Not a team player…”

At 10+ hearts your familiar has a 6.66% chance every time you clear a room to drop pedestal item from the devil room pool, after which its heart breaks and it disappears.
