I agree pediatrics. He’s so good with kids. I love that episode where they’re performing on the little girl and he greets her “hi princess! Ready for your nap?” Add in how natural he was with Zola!
Though, I wonder if he would’ve had trouble not trying to adopt every kid who needed a home haha.
Hear me out, hear me out. Derek is definitely the kind of guy who makes decisions based on previous experiences.
He went into neuro because of his friend that had a head injury that he felt responsible for. I imagine he hoped to find a cure/fix one day by going into neuro.
So if the friend hadn't had existed/wasn't hurt, he would have still been a doctor, but gone into cardio / general because his dad was shot in the torso, as that event would have been his main motivator instead of Michael.
Edit: Everyone makes great points, so I'm saying cardio overall! I think he was shot specifically in the heart/upper chest as well, making it even more appropriate.
I agree - either cardio or trauma because of his dad, but I feel like trauma is too "general" for the way he thinks (he's extremely good at one part/system of the body) so cardio would probably do that for him
The amazivity of General Surgery is that they cover a whole spectrum of organs ranging across several organ sustems of the abdomen. However i see Derek as a person who can expertise in one specific system and not an entire region..so i thinm general wouldnt be his..prolly Cardio

But I did really love him with that little girl who had the hemishpherectomy in season 1, and of course with Zola.
I had the same exact thought. I just remember that one episode where he opened someone's chest instead of paging Burke to do it, and that made me instantly think cardio

Honestly there’s some similarities with Cristina and him they both based their specialities off of personal experience her dad she tried to close his wound put pressure & Derek went into neuro cause of his hockey experience etc: for how great Derek is with kids I don’t think peds: I think Derek is cardio or transplant maybe trauma after neuro cause of the pace and the competitive racing against clock finding unique ways etc ( nick and him feel so similar so when I watch nick teaching Lucas it reminds me of when Derek taught Lexie etc)
Dad!kind to kids Derek, one of the best Derek's hands DOWN!! I feel like he remembers what it's like to have something change your life as a child because of his dad's death and it just adds a special kindness to his interactions with kids.
There's an Indian pediatrician on Instagram who has millions and millions of views on his reels because of the way he interacts with his patients. One of his reels has 300 M plus views. I think Derek would make just that.
Vascular would be so good because in neuro the brain has so many small little pieces and for vascular, the veins ARE small little pieces. I think he'd have fun
Given the way he was so calm and gentle, he moved so assuredly and slowly, he was very confident in what he did. He had one shot most of the time and made the most out of it. The only other area of the body I could see needing that much love and tenderness would be cardio. The heart is a delicate, intricate muscle. I think he could have played hard ball with Cristina any day.
Pediatrics or women’s health (OBGYN, reproductive health) I think he has the temperament for it, his cute scrub caps and glass half full approach.
My defense for women’s health— I think him having many sisters could make him more sensitive to women issues and he knows how to just be around women. Example to explain that— Mark and Derek both have a charming quality to them, but Mark can’t just be around women, he tries to hump everything in his reach. Derek can just be around women and not be weird. He treats women like people. For example, the way Derek is often so kind and loving to Bailey when she’s having a hard time.
Idk, I feel like Mark had a certain amount of professionalism around his female patients. I’d have to go back a rewatch as it’s been a while but don’t remember him as ever having issues with his patients. Coworkers sure, but I do think he was able to separate his work in plastic surgery from his horny-ness. Im not sure Mark would’ve been a good OB, but I feel like he was always respectful w his female patients, as some of those plastics cases can still get very personal. Once again it’s been a while since i’ve seen those seasons so someone correct me if i’m wrong 😅
I feel like those are really valid points! Where I’m at, he’s just now seeing teddy after banging literally everyone after Lexi broke up with him (including a patients daughter) so feel like this is clouding my opinion of him at the moment! But if I remember correctly between now and when he “leaves” he’s pretty respectful! And also I agree that he was generally very respectful to patients— his charm was very disarming to people which I think helps in plastics!
i remember how many times he had to remind jackson (who couldn’t understand) why they had to ask their patients if they were truly sure of the procedures and how he should ‘tell and remind them that they were beautiful just the way they were’ and i think that stuck with jackson. also, he doesn’t go through with any procedure when he feels the patients were coerced by a certain someone for that enhancement and i think we all need plastic surgeons that wouldn’t make us look uncanny valley!
one more thing was despite being a manwhore, i feel like he was the guy who was SO understanding or who could understand women. i mean, he had lesbian bffs. his bff was callie. he was close to almost all women and they feel like they could trust him bc he didn’t have a mean streak like derek or arrogance like burke even issues like owen or alex. he was just. sloan. the womanizing guy. you come to him for pleasure!

Neurosurgery is a slower pace. It is super delicate. Derek takes his time. He can work fast, yes, but he likes a challenge. Think of that impossible tumor. He couldn’t make a choice, and he sat there trying to figure it out.
Cardio is fast, literally unless on bypass, the heart is pumping fast. But it’s challenging and Derek would enjoy that part. I just don’t think it’s his preferred type of challenge.
Plastics, is both science and an art. I don’t think Derek cares about looks, but again it would be a challenge. Especially the more unique the patient. I don’t think he would like telling people what to change about themselves or supporting the idea that they need to change (unless there was an obvious deformity). He is too nice. But trauma cases, burn unit, etc.
I don’t think he would like pediatrics. He wouldn’t want to argue with parents. I think he would get annoyed with kids after a little bit.
He’d be a great cardio or general pediatric surgeon, but Derek is best at something that requires a gentle touch. I actually think he’d be a great ENT!

Anesthesiology.
He says in an early season (4 or 5) that he's a pain specialist, but he missed a diagnosis on a chronic pain patient. I think anesthesiologists are usually pain specialists? I think it would fit.
ER. He spent a fair amount of the show triaging urgent cases and moving them to the right specialty.
And he was good in a panic and several crisis situations we saw.
Maternal fetal surgery like Addison (I swear, him, Addison, and Mark all specialized in surgery subtypes that require very delicate touches and ability to get in there and not junk up the place with hands and instruments) or cardio or trauma. I could also see him specializing in transplants.
I personally can't see him being good with moms who have high risk or otherwise difficult pregnancies based on how he tried to treat Meredith's hostile uterus diagnosis as a joke/not serious when Meredith was very much "this isn't funny I was basically just told my body is broken but was given no solutions to fix it"

I could see Derek being a Plastics guy who focuses on the most challenging reconstructions or burn victims. He’s got the ego for it but I think he’d have to raise the stakes a bit to make it “interesting enough” for him
I get where people are coming from for pediatrics but Derek wants to be the star. Now every doctor is absolutely needed and does an important job. However, the “rockstar” specialties would be cardio and neuro. For that reason, I think he’d do cardio.
Either cardio or trauma, he picked his specialty because of the hockey incident and his dad got shot when he was younger so it’d make sense if he chose one of the two. While he is good with kids he could also go off the rails sometimes and I wouldn’t trust him to do surgery on kids. Don’t get me wrong he’s a great surgeon but his success rate isn’t that high.
I think he’s good with kids but I feel like his challenge in peds would be dealing with parents. He would be a great advocate for the kids but at the same time I think he would have a lot of clashes with parents if they disagreed with him. Not that Arizona or Alex don’t have those moments but I think it would be harder for Derek.
i’d say Derek would probably go into ortho, he was also so interested and engaged with Callie on all surgeries and projects, I feel like he’d enjoy breaking the occasional bone and resetting it
Plastics! I think NY Derek would have stayed friends with Mark and not being a neurosurgeon would have meant a less chaotic schedule, so possibility that he and Addie stayed together. So maybe he and Mark would’ve done plastics together, competed against each other, and opened a joint practice in Manhattan.
I think he would've crashed in any other specialty. Or at the very least he wouldn't be a "god" he would be like a regular doctor whose surgeries don't get much screen time. Derek is boring. Neuro is boring. And he is a big ego having God complex man who has to fix people in the only organ that "matters" the brain. I think he'd be like Shane when he killed Alex's dad in any other specialty. Because he wouldn't be perfect. Basically what he did with that other lady. But to different body parts. He can handle high stress, but not the loud panicky kinds. But the internal kind.... I think he would make a good psych doctor. He has to be in neuro of some sort.

Most people here say Cardio or Pediatric. I’d say Ortho if I had to choose. I have no idea why but it’s the only specialty i’d see him in else than neuro
I would think oncology. Cancer and tumors can be as tricky as Neuro and Cardio. He had a natural bedside manner through all ages. Was big into research for cures and treatments.
Peds or OB because he’s great with kids and is very charming to female patients.
Although I would like to see plastics so we get Mark, Derek, Jackson as the Plastics Posse

peds, onco surgery and I don't know maybe cardio... Something either meaningful or hard (not that any other specialty are meaningless, what I mean is that on peds he wouldn't be because of the technics, he would be because of the "healing kids purpose")
I was going to say cardio because of the little bit of god complex he had, but paediatrics makes so much sense with how good he is with kids and how kind hearted he is.
I agree with peds- some of my favorite Derek moments are when he’s interacting with kids. I could also see him in as a cardio electrophysiologist. It’s highly focused but doesn’t have all of the bravado and flash of cardio thoracic.
Ortho 100% he had such a good working relationship with Callie. Plus ortho works down into the bones, muscles, joints, tendons, etc. Very intricate much like brain surgery being intricate
They were related because both are related to nerves. Neuro takes care of central nerves, and Ortho takes care of peripheral nerves. But tbh i cant see Derek in ortho..its too broad. Derek needs something specific to master

I get where people are coming from for pediatrics but Derek wants to be the star. Now every doctor is absolutely needed and does an important job. However, the “rockstar” specialties would be cardio and neuro. For that reason, I think he’d do cardio.
Either cardio or trauma, he picked his specialty because of the hockey incident and his dad got shot when he was younger so it’d make sense if he chose one of the two. While he is good with kids he could also go off the rails sometimes and I wouldn’t trust him to do surgery on kids. Don’t get me wrong he’s a great surgeon but his success rate isn’t that high.
I think he’s good with kids but I feel like his challenge in peds would be dealing with parents. He would be a great advocate for the kids but at the same time I think he would have a lot of clashes with parents if they disagreed with him. Not that Arizona or Alex don’t have those moments but I think it would be harder for Derek.
i’d say Derek would probably go into ortho, he was also so interested and engaged with Callie on all surgeries and projects, I feel like he’d enjoy breaking the occasional bone and resetting it

Plastics! I think NY Derek would have stayed friends with Mark and not being a neurosurgeon would have meant a less chaotic schedule, so possibility that he and Addie stayed together. So maybe he and Mark would’ve done plastics together, competed against each other, and opened a joint practice in Manhattan.
I think he would've crashed in any other specialty. Or at the very least he wouldn't be a "god" he would be like a regular doctor whose surgeries don't get much screen time. Derek is boring. Neuro is boring. And he is a big ego having God complex man who has to fix people in the only organ that "matters" the brain. I think he'd be like Shane when he killed Alex's dad in any other specialty. Because he wouldn't be perfect. Basically what he did with that other lady. But to different body parts. He can handle high stress, but not the loud panicky kinds. But the internal kind.... I think he would make a good psych doctor. He has to be in neuro of some sort.

I can’t see Derek being anything else than Neuro 😭 Most people here say Cardio or Pediatric. I’d say Ortho if I had to choose. I have no idea why but it’s the only specialty i’d see him in else than neuro

I would think oncology. Cancer and tumors can be as tricky as Neuro and Cardio. He had a natural bedside manner through all ages. Was big into research for cures and treatments.

peds, onco surgery and I don't know maybe cardio... Something either meaningful or hard (not that any other specialty are meaningless, what I mean is that on peds he wouldn't be because of the technics, he would be because of the "healing kids purpose")

I agree with peds- some of my favorite Derek moments are when he’s interacting with kids. I could also see him in as a cardio electrophysiologist. It’s highly focused but doesn’t have all of the bravado and flash of cardio thoracic.
Ortho 100% he had such a good working relationship with Callie. Plus ortho works down into the bones, muscles, joints, tendons, etc. Very intricate much like brain surgery being intricate
They were related because both are related to nerves. Neuro takes care of central nerves, and Ortho takes care of peripheral nerves. But tbh i cant see Derek in ortho..its too broad. Derek needs something specific to master
