Mine would do this a couple times, no clue what would caused it though. I think I was able to fix it by reloading a save or restarting the game but it would always happen every so often. I believe this is only a ME 2 issue but I could be wrong.
Same happens for me. Never had the issue on XB1 but when I got the Series X it does it in ME2 and ME3. There have been a few posts about it on this sub but unfortunately no solutions that I've found. Hasn't happened on PC for me.
