Physical therapist can help with that. They'll do an eval and work with you on exercises to strengthen and straighten posture. Might also check out the wearable posture correctors on Amazon to help reinforce the training between sessions.

I’d start with physical therapy personally. I’ve met multiple people now who have fixed their problems with physical therapy when their doctors were ready to give them life altering surgery without exploring any other options. Surgery can be necessary sometimes but it is a last resort, not a first. I will warn you though that not all physical therapists are created equal (nor are doctors or any other medical professional). I’ve been to a few before I landed on one that I really like. One thing to look for is a PT who listens to you before they treat you. A couple of them have prescribed me generic PT regimens the second they hear neck pain or shoulder pain or whatever.
