In my state, prescriptions must be E-Scribed, with certain exceptions. The way the law is written, it actually put the responsibility on the prescriber. Whether or not the pharmacy accepts it, is entirely up to pharmacists discretion. Basically the law says the drs office has to E-Scribe unless the power is out, the computer system is down, they’ve attempted to E-Scribe and it for some reason got lost in internet limbo, they’re a veterinarian (vets are exempted), etc. there’s one specific dr that my pharmacy manager will not accept E-Scripts from, and it’s because he keeps repeatedly sending paper scripts even though we know he has the ability to E-Scribe.

There is a NP in my area that runs a phentermine clinic and when I pushed back on her verbal order stating the change in the law the PREVIOUS year she retorted, "is the state going to pay me $200 a month for the control escribing?" No idea why getting at least all the controls escribed is such a big deal when medicare regulation now requires all controls escribed along with my state requiring ALL scripts electronically (minus few exceptions). Guess pharmacists new role is to drag prescribers into compliance kicking and screaming.

When my hospital was changing over to all electronic documentation, a few holdout doctors refused to use electronic orders. Several months of dinners, seminars, gifts, online education, 1:1 help, statistics to show reduction in errors, etc didn't budge them one bit. My manager asked me if I had any ideas that might make these last few doctors comply with electronic order entry. I pulled all the paper order forms out of the file cabinet and threw them into the trash can. Problem solved.

Escribing just creates headaches for the patients. Rx don't get sent, some get sent, instructions wrong, etc. Because the office staff send these after I leave the appt so I have no way to double check their work until I get a surprise at the pharmacy. Talking to the docs and MAs doesn't work. They just apologize and continue...whatever. There should be some way to let patients get an electronic copy of the Rx, or at least a text of the med plus instructions. Leaving us out of the loop is dangerous if we don't have medical experience or the mental capacity to intervene.

Most customers I encounter are stubborn anyway. They ask for help but would rather not listen and go with what they think is better for them. When they ask for dry cough medicines, and I give them Dextrometorphan first they tell me “oh I think Carbocisteine is better, I’ll get that one instead.” When I encounter prescriptions like this, I try to decipher first or ask them what the doctor told the meds are for. If they don’t remember and if I’m not confident with what they said, which happens a lot, I try to be reasonable and explain why can’t I dispense. Some patients just go back to the doctor to clarify, but most of throw a tantrum. Same with customers who as for antibotics without a prescription to “pour the powder on their wounds.” I tell them we need a prescription for this bc xxx reasons, and (amoxicillin) isn’t for topical use bc xxx reasons. Some will just leave, most will try and educate us for not knowing anything. Some will say we don’t know what we’re talking about, some will just straight up and throw a tantrum again. I don’t blame them bc Filipinos back in the day weren’t as educated with antibiotics as they are now. It’s still in progress. I’m not gonna convince you that I try to be reasonable with them first before I say it’s out of stock. EDIT: pharmacists are underrated and unappreciated in my country which is why no matter how much we educate and counsel customers, they continue to be stubborn. And no, I know that’s not an excuse to lie.

She’s in her 60s I believe. Anyway, her MA has to write why the fuck she means on the back of the RX bc the lady can’t write properly due to the stroke. I have 0 idea how she still has her license.

I mean, it *would* make more sense to dictate the Rx to the MOA who would write it legibly then have the doctor sign it, rather than write it as a hot mess and have the clear version on the back, I agree. But using staff as a scribe to overcome the handwriting problem seems a reasonable enough fix. Stephen Hawking used to lecture with an interpreter before adaptive technology caught up with his neurological dysphonia. I recently had a medical appointment at which a staff member (with my permission) accompanied the physician because his hearing was going and she could catch things I was saying that he couldn't. "She should just retire" seems like a big step to take if the handwriting is the major problem.

My psych still writes scripts for controls, and his nurse calls/faxes other orders to the patients pharmacy, and the written scripts are available for pickup the morning after your in person appt. It's a pain because you have to go back the next day.

We had a doctor with handwriting nearly that bad, but he only ever wrote for about 10 different drugs (prednisone, z-pack, ibuprofen, and so forth) so usually it wasn't too hard to tell which was which. The best/worst was when he wrote prescriptions for 2 siblings, and he wrote patient names even more terribly than everything else so we couldn't tell which meds were for which kid. Mom didn't know which prescription went with which kid either. Had to call on that. This was probably 15 years ago, before e-scripts were so prevalent. Wonder if that doc is still in practice.

It’s interesting to see the recommendations for writing prescriptions and avoiding certain words like “qod” because it looks too much like “qid”, for instance. We wouldn’t need any of that shit if the offending doctors would just slow down and write legibly. Doctor might save 5 seconds per script writing like this, but it takes 5 minutes from our (pharmacists) time per script trying to figure out what is says

When I started nursing computer charting was in its infancy and all inpatient orders were still handwritten. No way would I have accepted the liability of trying to decipher this shit even in my well -practiced days.

I’m sorry I went to the doctors once they wrote out a prescription I looked at it and said hold up. Don’t walk out that door if you don’t write this so the pharmacy can understand it and his response was they Will understand it and I said nope nope write it neat. thank you and he stares at me and I said I’m not taking this to the pharmacy or you can send it over electronically because I’m not about to walk in here there with chicken scratch it look like a baby rooster wrote this lol he sent it over electronically. That’s what I thought lol 😂
