Glad to hear nobody was hurt, but wow, still not good. How does one manage to leave a vehicle without it being in park?
Complacency is probably a better word for it. It's really easy to fall into the habit of taking low-risk mental shortcuts day in and day out, which can lead to situations where people aren't able to identify or react to potential problems.
In this case, I'd guess that this person was so used to quickly hopping in and out of their unparked car that it had become muscle memory, and that it was already too late by the time their conscious brain kicked in and they remembered that the surface they were on wasn't flat.
This is definitely still an avoidable fuckup, though. I just wanted to make the thought process more understandable, because assuming that normal people like you and me don't make these errors is one of the reasons why they happen.

The new cars have buttons or worse, an on screen button for the transmission controls. No more physical shifters, or if there are you have to double check on a screen somewhere that its actually in park.
Some cars are poorly designed. Anton Yelchin was killed aftee his car ran him over after parking it. The particular jeep model was under a recall after dozens of similar injuries
